In order for your student to begin participating in the afterschool BAM program, we kindly ask that you complete and submit the  enrollment form. Once we receive the completed form, we will contact you by phone to confirm your student(s) start date. If you have more than one student being enrolled in the program please have separate enrollment forms for each student.

Please note that at the moment if you are requesting bus transportation, there is a delay. In order for your student(s) to start BAM they would have to be parent pick ups until we are able to accommodate your request.

Para que su hijo/a pueda comenzar a participar en el programa BAM extraescolar, le solicitamos que complete y envíe el formulario de inscripción. Una vez recibido, nos pondremos en contacto con usted por teléfono para confirmar la fecha de inicio de su(s) hijo(s). Si tiene más de un estudiante inscrito en el programa, tenga formularios de inscripción separados para cada estudiante.

Tenga en cuenta que, por el momento, si solicita transporte en autobús, hay una demora. Para que sus estudiantes comiencen BAM, deberán ser recogidos por sus padres hasta que podamos atender su solicitud.
